When you sign up to become a member or otherwise become a patient of Longevity Club + Clinic, we collect information concerning your physical health, health history, medications, and other related health information. The purpose of this collection is to provide you with health services that you request.
How do we use your health data?
We use your health data to provide you the healthcare Services you request, including but not limited to IV therapy, peptide therapy, hormone optimization, hyperbaric oxygen therapy, red light therapy, EBOO/ozone therapy, and comprehensive health assessments.
Who do we share your health data with?
We share your health data with third parties that are involved in providing healthcare services to you as a patient and parties that may be providing billing-related services.
